Payment is due at time of service. We accept Cash, Credit and Debit Cards. We also accept Flex Spending Insurance Cards.

 
Those with Insurance:
HOPE Family Medical Center now accepts Cigna Health Insurance, and will be accepting new patients with other health insurance in the very near future. We will update this page as soon as we receive our certification with each insurance carrier. Please check back frequently to view updates. Meanwhile, patients who have medical insurance with an out of network benefit and/or who have a flex spending account can still save on services received at our clinic. For those with an out of network benefit you will receive a comprehensive statement to submit to your health insurance carrier for reimbursement. And, if you have a flex spending account you can use your card to pay for all your medical needs at the HOPE Family Medical Center Clinic. Please speak to our billing specialist about how Hope Family Medical Center can help you and your family.

Medication Policy:

Narcotic Pain Medications:

We believe that narcotics are good for a limited number of short term medical problems however we do not prescribe narcotics for long term use.  

We DO NOT prescribe medication(s) for chronic pain conditions, however we will gladly refer you to a pain specialist or pain clinic for your chronic pain needs.

If you are taking prescription pain medications or other controlled substances prescribed by another medical provider you will need to contact that provider for your prescription refills.

Prescription Refills: 

If you are a patient of HOPE Family Medical Center we will be happy to refill prescriptions that we prescribe. 

New patients will need to transfer their medical records to the HOPE Family Medical Center to establish continuity of care before we can refill prescriptions prescribed by other medical providers. we can refill prescriptions prescribed by other providers.

Controlled Substance Refills:

New patients will need to transfer their medical records to the HOPE Family Medical Center before we can consider refilling controlled prescriptions that are prescribed by other medical providers. We will not refill controlled substance prescriptions at you first clinic appointment.  

We will also order a controlled substance (DOPL) report for all patients who are on a controlled substance medication prescribed by another provider and/or if you request a prescription for a controlled substance medication.
